• the fact that they have had the state-mandated Hepatitis B vaccine series or are making progress toward completion of the series, and
• the fact that they have read the Meningicoccal Vaccine Sheet.
• the fact that they have read the Measles, Mumps ,and Rubella Sheet(MMR).
  

Read the Meningicoccal Vaccine and Sheet and the MMR Sheet, submit the online Immunization Requirement Form.

Note: The Hepatitis B and Meningicoccal Vaccines are available at the Student Health Services. You are welcome to make an appointment to receive them up to 3 month in advance of your initial enrollment date. Call (951) 827-3031 for pricing information and to make an appointment.
